For over 30 years, Rehab Without Walls® has served thousands of people with our Home and Community (H&C) rehabilitation programs. Our approach blends evidence-based therapies with creative uses of the patient’s surroundings, including home, school, work, and more. Providing rehab in a home and community environment offers a huge advantage in treatment. We address patient goals and barriers in real-life settings to help increase motivation, inspire durable outcomes, and reduce the risk of injuries that could lead to rehospitalization.

Home & Community neuro therapy is best suited for children and adults who:

• Are medically stable enough to return home
• Have appropriate support for safe transition to home and/or have limited access to transportation to facility-based programs
• Will experience a clinical advantage, such as functional goals requiring practice in their own home/community setting

Our Approach:
Our primary focus is on teaching relevant skills in the individual’s environment as opposed to a staged setting. We go beyond personal care and take it a step further into real-life experiences such as navigating the kitchen, bathroom, stores, and workplaces. We also prioritize the individual’s personal goals when creating therapy plans.

Our Care Team:
Each patient in our H&C rehab program will receive an individualized therapy plan that incorporates elements of the individual’s home into therapy. Our H&C care teams are overseen by a physician and led by a clinical coordinator who ensures there are no gaps in services, leads team meetings, and more.

Our Comprehensive Evaluation:
Upon admission, a comprehensive evaluation is conducted to identify safety issues in both the home and local community. We conduct the following evaluations: accessibility, emergency preparedness including the ability to exit the home in the event of an emergency, fall risk, equipment needs/appropriateness of existing equipment, caregiver support, and more. Our evaluation includes understanding our patient’s cultural beliefs, learning needs, telecommunication abilities, and life stressors as these are pivotal to successful outcomes.
